OCD in children: signs parents can notice

Obsessive compulsive disorder (OCD) can start in childhood, as early as around age 6. Children may repeat rituals such as washing, checking or counting, or become very distressed when things are not done a certain way. It is treatable, and therapy helps.

Signs in behaviour

  • washing hands again and again
  • checking doors, bags or homework repeatedly
  • counting, tapping or touching things in a set way
  • asking for the same reassurance over and over
  • taking a very long time over simple tasks

Signs in thoughts and feelings

  • fears of germs, harm or something bad happening
  • distress when things are out of place
  • worries or images the child finds upsetting and cannot dismiss

What parents may notice

Children often hide their rituals out of embarrassment. Look for sore or dry hands from washing, slow homework, a drop in school performance, or a child who is anxious and tired.

Treatment

Talking therapy, usually cognitive behavioural therapy (CBT) with exposure and response prevention, helps a child face the fear without doing the ritual. A psychiatrist may suggest medicine for some children. Family support matters too.

When to get help

See a doctor if rituals take up time every day, cause distress or interfere with school and family life.

Common questions

Is OCD just being neat and tidy?

No. OCD involves unwanted thoughts and repetitive actions that cause distress and take up time. Liking things tidy is not OCD.

Should I give reassurance if my child keeps asking?

Reassurance often gives short relief and keeps the cycle going. A therapist can show you how to respond.

Can children recover from OCD?

Many children improve a great deal with therapy, and sometimes medicine. Early help is best.
